Senior Care Program
This program provides case management and funding to seniors at risk of nursing home placement. Eligibility requirements: Must be at least 65 years of age and meet income and assets requirements.
Program Services may include:
- Case Management Services
- Financial Assistance for Medications
- Financial Assistance for Incontinent Supplies and Nutritional Supplements
- Person Emergency Response Systems
